Sara Ahmed is a clinical epidemiology researcher affiliated with the RI-MUHC. She is the Scientific Director of Québec’s Digital Health Network and co-directs the McGill Centre for Health Measurement. She also leads the BRILLIANT Learning Health Community, part of an international network focused on advancing learning health systems in rehabilitation.
Her work centres on designing, implementing, and scaling digital health solutions, and integrating patient-reported outcomes and experiences into routine care. She aims to support more equitable, evidence-informed health systems that deliver meaningful value for patients, providers, and communities, particularly those living with chronic conditions. Her research also emphasizes collaboration across disciplines and the use of data to continuously improve care.

Discover how digital health is transforming rehabilitation care and improving patient pathways. This conference highlights an innovative approach to integrated health systems, where patients, clinicians, and decision-makers have access to real-time information to make informed decisions at every stage of the care journey.
The presentation introduces BRILLIANT (Biomedical Research and Informatics Living Lab for Innovative Advances of New Technologies), an innovative digital platform designed to optimize care transitions, enhance communication between care teams, patients, and their families, and support better coordination of interventions. It also enables the measurement of outcomes that matter to patients, the personalization of care plans, and the provision of tools that promote self-management.
Through a collaborative approach involving patients and health system stakeholders, this initiative demonstrates the real potential of digital solutions to improve user experience, quality of care, and clinical outcomes.
